Application Instructions

(Please have all needed information, together and ready to upload, before submitting application.)

 

Please fill out the application in its entirety. Applicants will be notified by email whether their child has been accepted into the STEAM Honors Program, so please be sure to include your correct email address.

 

Parent/Guardian

 

Part 1 – Student Information (to be completed by the parent/guardian).

 

Part 2 – Parent/Guardian Perceptions (to be completed by the parent/guardian).

 

Part 3 – Student Essay (to be completed by the student and files uploaded by parent/guardian.) The STUDENT needs to write in their own words and handwriting the answers to the questions. The PARENT/GUARDIAN needs to take a picture/copy of the student response and upload the file. (These responses need to be in the child's own words and handwriting.)

 

Part 4 – Teacher Recommendations. Two teachers are required to submit a questionnaire. A link will be sent to the teacher, based on the email address provided by the Parent/Guardian. Please contact your child's teacher and make them aware you have given them as a recommendation. (A current teacher, along with one other teacher who interacts with your child on a regular basis.)

 

Part 5 – Parent Agreement (to be completed by the parent/guardian).

 

Teacher

 

A link will be sent to the teacher, based on the email address provided by the Parent/Guardian. The teacher will fill out the form and submit online to Bartlett City Schools.

 

Performance Rubric

 

This rubric will be filled out by the Teaching and Learning Department at Bartlett City Schools. The performance rubric will be populated with student grades, benchmark assessment data, and test scores. If the STEAM Honors Program applicant is new to the district, please provide any student performance records you can obtain, such as report cards, benchmark assessment data, and test score reports.

 

Notification Process

All applications will be evaluated. Parents/Guardians will be notified by email whether your child has been accepted into the STEAM Honors Program. Students who qualify, but are not accepted due to space, will be placed on a wait list and will be notified as space becomes available.
